Join us to summarize another fantastic series of Grand Rounds lectures. We start off with in-flight emergencies, including changes to physiology and what equipment is available to you on most commercial flights, with Dr. Urbanowicz. Followed by a literature-filled debate between Drs. Lane and McDonough on the utility of CT pan-scans in trauma patients. With the help of Dr. Shewakramani, we all learn how to better recognize and care for patients with sepsis. Meanwhile, Dr. Roche teaches us about all various bites/stings- including marine species, scorpions, and mosquitos! Lastly, Dr. Wright introduces us to the science of reliability and how to design processes in healthcare to be more reliable, with the ultimate goal of improving our care of patients.

We had an exciting Grand Rounds this week. We started off with our visiting lecturer, Dr. Jennifer Wiler, who gave us an excellent overview of the dollars and cents behind the care that we provide in the emergency department. Afterwards we practice some oral board style cases. Dr. Betz led a challenging triple patient encounter where we had to simultaneously manage a knee dislocation, aortic dissection with involvement of the right coronary artery, and pneumonia in the setting of HIV. Dr. Curry led a riveting case of preeclampsia where identifying the critical history of a recent delivery was critical to initiating the correct management. We wrapped up the day with a simulation led by Drs. Hill, LaFollette, and Lang on in-flight emergencies!

Severely burned patients can be intimidating for even the most seasoned critical care transport providers. These patients often require aggressive resuscitation and multiple procedures in a relatively short period of time. It is often easy for providers to become overwhelmed, necessitating an algorithmic approach to the patient, similar to traumatically injured patients, is crucial. By advancing through the primary survey and stabilizing the patient while starting aggressive but goal directed crystalloid resuscitation, critical care transport providers can bring ICU level care to one of the sickest pre-hospital patient populations.

It is mid July and your first shift as the coveted H2 Doc at Air Care 2 is finally upon you.  It has been an especially warm and beautiful Saturday and you ponder the possible flights for the evening as you take the scenic drive to Butler County Regional Airport.
