Dr. Ludmer kicked things off with this month's Morbidity & Mortality conference where we discussed C. Diff, spinal cord compression, LP in the setting of anticoagulation and cardiac arrest with an LVAD. Dr. Stettler led the next segment of our longitudinal leadership curriculum with a look at the mentor/mentee relationship. We finished up with a soapbox lecture by Dr. Titone on conflict resolution and an R3 follow up by Dr. Bernardoni who discussed the crashing patient with right ventricular failure and pulmonary arterial hypertension.

In this week's Grand Rounds, we led with our first Morbidity and Mortality conference of the year. Dr. Lagasse walked us through cognitive biases, the management of early obstetric emergencies, and the management of skin and soft tissue infections in the diabetic foot. In our leadership curriculum, Drs Hill and Stettler discussed the fundamentals of being a leader, and we got to hear from a panel of accomplished physicians within our department about their own leadership journey. Finally, in our Consultant of the Month series, Dr. Martha Ferguson discussed the management of ano-rectal conditions in the Emergency Department.

Dr. Ashley Shreves from Ochsner Medical Center led off Grand Rounds this week with a great lecture on why Emergency Medicine physicians should be good at Palliative Care and then taught us a step-wise approach to code status discussions in the Emergency Department. Drs. Ronan and Kreitzer brought us the latest installment of the Leadership Curriculum, where we learned about and practiced different styles of communication.

Dr. Betham started off Grand Rounds with this month's Morbidity and Mortality Conference, where we learned about Gout, ITP, Stroke, and Hyperkalemia in ESRD patients. Dr. Teuber walked us through the varying recommendations regarding ED Thoracotomy and Dr. Banning taught us how to handle neonatal urgencies. Leadership Curriculum wrapped up this week's conference with a session on how to pitch an idea.

We were reminded about patient literacy and the advantages of keeping it simple. Dr. Hill also taught that every shift our biases and decision making do affect patient care, it is our duty to recognize and use them to our patients' advantage.

Evidence-Based Emergency Medicine: Accidental Hypothermia with Drs. Mudd & Riddle

Grading the Severity of Hypothermia

  • Mild hypothermia is defined as 32-35 °C and symptoms include confusion and diuresis
  • Moderate hypothermia occurs from 28-31°C and is associated with lack of shivering, atrial arrhythmias, and worsening changes in mental status (including paradoxical undressing)
  • Severe hypothermia happens when core body temperature is less then 28 °C and is associated with coma, significant decreases in metabolism, and a very low threshold for V-fib
